Job Summary
Conduct in-person and telehealth visits for 45-60 patients in home settings, assisted living, or skilled nursing facilities. Build meaningful relationships with patients and families by creating care plans centered on their wishes, needs, and beliefs, often collaborating with primary care physicians. Advocate for patient goals while educating caregivers and communities on compassionate end-of-life care. Participate in weekly interdisciplinary team meetings with social workers, chaplains, and nurses to provide holistic support. Manage cases through an interdisciplinary team approach while maintaining board certification in hospice and palliative medicine, internal medicine, family practice, or related specialties.
Required Qualifications
- Licensed to practice medicine or osteopathy in the state in which their program operates
- Board-certified or board-eligible in hospice and palliative medicine, internal medicine, family practice, or a related specialty
- Registered with the DEA
