Job Summary
Physician in home hospice care responsible for ~45-60 patients seen in patients' homes, with in-person and telehealth visits, collaborating with an interdisciplinary team, participating in weekly IDG meetings, developing individualized care plans aligned with patient goals, and educating patients, families, and communities on end-of-life care; must be licensed in the operating state and board-certified/eligible in hospice/palliative medicine, internal medicine, or family practice; role emphasizes compassionate, patient-centered end-of-life care across private residences and facility settings; strong commitment to extending comfort and dignity throughout the patient’s life journey.
Required Qualifications
- Licensed to practice medicine or osteopathy in the state of the program
- Board-certified or board-eligible in hospice and palliative medicine, internal medicine, family practice, or related specialty
- Registered with the DEA
