Physician Home Care

Physician provides home hospice care to patients and families, coordinating with an interdisciplinary team to deliver patient-centered end-of-life care. Responsibilities include managing 45-60 patients in home settings, conducting in-person and telehealth visits, participating in weekly IDG meetings, and educating patients and caregivers about hospice benefits. Requires state medical/osteopathic license, board certification/eligibility in hospice and palliative medicine, internal medicine, or family practice, and DEA registration. Employment location at Kissimmee, FL with care delivered in patients' homes or related facilities; occasional visits to local program office for case discussions with the team.
