Physician for Skilled Nursing Facilities - Primary Care West Haven - Branford

Provide and manage direct patient care, including physical examinations, evaluations, assessments, diagnoses, and treatment for a specified population under indirect supervision. Engage patients and families in setting self-management care plans, document individualized care plans, and coordinate activities with nursing and support staff. Analyze medical history and conditions to prescribe medications and refer patients to specialists. Respond to emergency situations, participate in quality improvement meetings, and follow infection control standards. Requires a Medical Doctor with 3 to 5 years of direct patient experience, full-time 40-hour schedule, and proof of annual flu vaccination.
