Job Summary
Family Practice Physician provides primary care services through identification, management, and/or referral of health problems and preventive care. Responsibilities include high-quality patient care with timely medical documentation, education of patients about diagnoses and treatments, coordination with other disciplines, and ensuring credentialing/privileging and compliance with policies. Clinicians rotate on-call for evenings, weekends, and holidays; participate in CME programs (up to 40 hours/year); maintain electronic medical records; and contribute to quality and risk management efforts. Requires U.S. medical school graduate status, completion of an ACGME-accredited Family Practice residency, board certification or eligibility, Arizona medical license, DEA certificate, CPR/AED certification, and a Level One Fingerprint Clearance Card. Bilingual English/Spanish and experience in community health are preferred.
Required Qualifications
- Graduate of an accredited medical school program
- Completion of an accredited United States Family Practice Residency program
- Board certified or board eligible
- Valid Arizona medical license
- Valid DEA certificate
- Demonstrated understanding of clinical methods and techniques
- Ability to meet and deal effectively with clients, associates and the general public
- Basic business English, spelling and punctuation skills
- Basic computer efficiency
- Valid AZ driver’s license
- Certification to perform CPR and AED by American Heart Association/Red Cross
- Valid Level One Fingerprint Clearance Card for behavioral health locations
