Job Summary
Conduct primary care services including chronic disease management, Medicare wellness, physicals, well-child examinations, acute care visits, and minor in-office procedures. Collaborate with the population health team to emphasize quality outcomes and support value-based care initiatives. Work Monday through Friday with occasional Saturday coverage and light phone call consults only. The role is within a physician-led group in Queen Creek, AZ, supported by medical assistants, specialists, a Pharm D, and an onsite lab.
Required Qualifications
- Board-Certified or Board Eligible in Family Medicine or Internal Medicine
- Monday through Friday, with occasional Saturday
- Light call: phone call consult only
- Have experience with or strong interest in value-based care, quality improvement, and population health

- Prefers to see adult patient population
