Job Summary
Provide acute, chronic, and preventive healthcare to patients across Essentia Health's six facilities in the Fargo and Moorhead area, working collaboratively with MDs, NPs, PAs, and ancillary staff. Manage a caseload of 36 patient contact hours weekly, seeing an average of 18–20 patients per day. Operate under a rotating after-hours call schedule with day/eve shifts running from 0800 to 1700. Lead a Care Team focused on patient and family-centered care while utilizing Essentia Care Processes to improve overall community health.
