Job Summary

Provide and manage direct patient care, including physical examinations, evaluations, assessments, diagnosis, and evidence-based treatment. Prescribe pharmaceuticals and treatment regimens, review medical records for compliance, and counsel patients on wellness and prevention. Supervise advanced practitioners, medical students, and residents; coordinate nursing and support staff activities. Refer patients to specialists and adhere to safety, infection control, and PPE standards. Participate in meetings, trainings, community outreach, and continuing education. Requires an active Arizona State Physician License, Board Certification in Family Medicine or Internal Medicine, and completion of accredited residency.

Required Qualifications

  • Internship and Residency completed in an accredited Family Medicine and Internal Medicine Residency Program (M.D./D.O.)
  • Arizona State Physician License, active and in good standing
  • Board Certified or Eligible in Family Medicine or Internal Medicine (ABFM or AOBFP; ABIM or AOBIM)
  • Basic Life Support (BLS) certification
  • DEA
  • X waiver
  • Ability to pass pre-employment background check and drug testing
  • Current Level 1 Fingerprint Clearance Card or must pass fingerprint clearance within 7 days following hire

Desired Qualifications

  • Ability to speak English and Spanish preferred
  • Ability to prescribe Medication Assisted Treatment (MAT)
