Physician in Family Medicine splitting duties between Logansport and Peru, IN with 100% outpatient care in a team-based collaborative model. Flexible schedule with no weekends or evenings, onsite integrated behavioral health, and eClinicalWorks EMR. Required Qualifications
- Doctor of Medicine (M.D.) or Doctor of Osteopathy (D.O.) from an accredited school of medicine
- Completion of an accredited Residency Program
- Licensed or eligible for licensure in Indiana
- Board-Certified or Board-Eligible
- Active and unrestricted DEA license or ability to obtain prior to start
