Job Summary
Provide continuing care for the individual and family across all genders, diseases, and body parts, emphasizing disease prevention, wellness, and health promotion. Deliver personal, comprehensive, and continuing care within the context of the family and community. Manage shared call responsibilities as a Board Eligible or Board Certified provider in a team-centered environment. The role supports a regional population of 220,000 in Northern Indiana with a strong Accountable Care Organization focus on population health strategies.
Required Qualifications
- Board Eligible or Board Certified
- Must be able to provide continuing care for the individual and family across all genders, diseases, and parts of the body
- Must be able to focus on community, emphasizing disease prevention, wellness, and health promotion
- Must be able to provide personal, comprehensive and continuing care for the individual in the context of the family and the community
- Must be available for shared call
