Job Summary
Manage a primary care practice with an average of 18 patients daily, operating within a 15-provider team at M Health Fairview Clinic - Burnsville. Deliver full scope or adult-only care under a multidisciplinary model integrated with the adjacent Fairview Ridges Hospital. Participate in a shared call rotation of 1:22 and maintain daily contact hours aligned with FTE status. Work within Fairview's Accountable Care Organization to focus on quality outcomes, care coordination, and population health management using fully integrated EPIC electronic medical records.
Required Qualifications
- Graduate of an accredited ACGME/AOA Family Medicine residency training program
- Board Certified/Board Prepared with the American Board of Medical Specialties or American Osteopathic Association
- Licensed or ability to obtain Minnesota Board of Medical Practice Licensure
- Ability to obtain and/or maintain DEA certification for the State of Minnesota
- Meet threshold criteria/qualifications for Credentialing and Privileges
