Physician - Chair of Care Team Innovation, Primary Care

Establish strategic vision and priorities for primary care transformation, serving as a senior clinical mentor to newly recruited and early-career physicians while championing adoption of evidence-based best practices. Lead efforts to expand collaborative practice arrangements with nurse practitioners and physician assistants, facilitating forums and coaching to accelerate leading practices. Partner with operational teams to enhance patient access, panel management, and care team optimization, utilizing data analytics to improve population health outcomes and guide clinical decision-making. Monitor provider performance, establish key performance indicators, and drive adoption of standardized care pathways under value-based and risk-based models. Maintain clinical FTE status and board certification in family medicine or internal medicine.
