Job Summary
Conduct in-home health evaluations lasting approximately 45 minutes, including medication and medical history reviews, physical assessments, and diagnostic tests such as spirometry or diabetic eye exams when ordered. Utilize an iPad and clinical workflow for each visit while connecting one-on-one with members to bridge care gaps. Provide all Signify Health services including preventive diagnostics, perform virtual visits as needed, and travel locally within licensed states. Work part-time with anticipated weekly hours of 40, covering Saint Cloud and surrounding areas. Candidates must be comfortable performing duties in diverse home environments that may contain pets or allergens.
