Job Summary
Conduct in-home health evaluations lasting approximately 45 minutes, including medication and medical history reviews, physical assessments, and diagnostic tests like spirometry or diabetic eye exams when ordered. Utilize an iPad and clinical workflow to provide comprehensive, personalized care while connecting one-on-one with members in their private residences. Fulfill all Signify Health services, including preventive and diagnostic offerings, and conduct virtual visits as needed. Travel locally and across licensed states to cover the Brainerd/Duluth/Minneapolis area, maintaining flexibility for on-call rotations. Work full-time hours with active, unrestricted licensure and board certification required.
