Job Summary
Conduct In-Home Health Evaluations lasting approximately 45 minutes, including medication reviews, physical assessments, and diagnostic tests like spirometry or diabetic eye exams. Perform virtual visits as needed while using an iPad-based clinical workflow. Travel locally and across licensed states to visit health plan members in their private residences, providing Diagnostic and Preventive Services. Maintain flexibility for on-site duties in diverse home environments. Work part-time with an anticipated weekly schedule of 29.5 hours.
Required Qualifications
- Active, unrestricted license(s) in coverage area(s)
- Completion of 2080 hours of post-graduate collaborative practice hours as an NP or PA in an integrated clinical care setting
- Filing of the appropriate attestation with the state of Minnesota
- Board certification
- Multi-state licenses
