Job Summary
Part-Time Clinician (Nurse Practitioner or Physician Assistant) to provide in-home health evaluations for Signify Health, with 40 hours weekly expectation and ability to travel locally. Responsibilities include conducting 45-minute home visits with medical history review, physical evaluation, and, if ordered by the health plan, additional diagnostic tests (spirometry, DEE, bone density screening, etc.). Use of an iPad and clinical workflow; prescribing, ordering labs, or altering current regimens not required. Virtual visits may be conducted as needed. Join a national network of clinicians delivering personalized health evaluations at patients' homes and connecting members to appropriate care. Primary locations: Augusta, ME and Cedar Rapids, IA. Requires active, unrestricted licenses and board certification; multi-state licenses preferred. Pay range is $37.66-$81.11 hourly, with annualized salaries approximately $78,333 to $168,709 depending on factors like geography and experience.
