Part-time Clinician (Physician Assistant or Nurse Practitioner) to provide in-home health evaluations in customers’ homes, including a medication and medical history review and a physical evaluation; may include diagnostic tests (e.g., spirometry, bone density screening) as indicated by health plan; virtual visits as needed; utilizes an iPad and clinical workflow; role does not require prescribing or altering current treatment; travel locally and within licensed states as needed; helps connect members with appropriate care in a national clinician network.
