Job Summary
Part-time Clinician (Nurse Practitioner or Physician Assistant) to provide In-Home Health Evaluations, visiting people in their homes and bridging gaps in care. Responsibilities include 45-minute visits with medication and medical history review, physical evaluation, and optional diagnostic tests (e.g., spirometry, eye exams) as ordered by health plans; use of an iPad and clinical workflow; conducting virtual visits as needed; travel locally within licensed states; join Signify Health’s national clinician network to improve health outcomes and connect members with the right care. Requires active license(s), board certification, and multi-state licenses (preferred).
