Job Summary
Physician Assistant or Nurse Practitioner in a full-time clinician role providing in-home health evaluations. Deliver 45-minute visits at members’ homes, performing medication and medical history reviews, physical evaluations, and, when ordered by the health plan, additional diagnostic tests (e.g., spirometry, eye exams). Use an iPad-based clinical workflow; no prescribing, lab orders, or treatment changes required unless specified. Travel locally within licensed states, conduct virtual visits as needed, and join a national network dedicated to improving health outcomes by delivering personalized care at patients’ doors.
