Physician Assistant

Part-time clinician (Nurse Practitioner or Physician Assistant) to perform in-home health evaluations for Signify Health (CVS Health). Visit patients at their homes for ~45 minutes, including medication and medical history review, physical evaluation, and optional diagnostic tests (e.g., spirometry, bone density screening) as ordered by health plans. Use an iPad and clinical workflow; no prescribing, no lab orders, or alteration of current treatment required. Role involves travel locally within licensed states, and virtual visits as needed. Primary duties include engaging with members to connect them with appropriate care and delivering diagnostic and preventive services as indicated (spirometry, DEE, etc.). Requires active licensure in coverage areas, board certification, and multi-state licenses (preferred). Anticipated weekly hours: 40 (part-time). Location constraints include Augusta, ME and Indianapolis, IN with in-person remote arrangements. Benefits include malpractice coverage, supplies, and a comprehensive benefits package.
