Physician Assistant

Part-time Clinician (Nurse Practitioner or Physician Assistant) to perform In-Home Health Evaluations for Signify Health, traveling locally to patients’ homes and conducting 45-minute visits that include medication and medical history review, physical evaluation, and optional diagnostic tests ordered by the health plan (e.g., spirometry, eye exams, bone density screening). Responsibilities include building trusted relationships with members, delivering diagnostic and preventive services via an intuitive iPad-based workflow, and conducting virtual visits as needed. Requires active licensure in coverage area(s) with board certification; ability to cover primary locations such as Augusta, ME (and Arlington, VA as indicated) and to travel locally within licensed states; flexibility to travel locally and within licensed states; part-time schedule with anticipated 40 hours per week as listed.
