Job Summary
Conduct in-home health evaluations across a defined coverage area, performing 45-minute visits that include medication reviews, physical exams, and diagnostic tests like spirometry and diabetic eye exams. Utilize an iPad clinical workflow to connect one-on-one with patients while reviewing medical history and answering health questions. Fulfill all Signify Health services including preventive screenings, with flexibility to travel locally, obtain overnight stays, and maintain multi-state licenses as dictated by business needs. Perform duties in diverse home environments, including those with pets or allergens, while conducting virtual visits as required.
Required Qualifications
- Active, unrestricted license(s) in coverage area(s)
- Board certification
- Able to obtain and maintain multi-state licenses in coverage area, as required
- Able to cover all counties within state and flex to other areas of business need
- Must be able to comfortably and safely perform duties in homes that may contain household pets, secondhand smoke and other potential airborne allergens or irritants
