Job Summary
In-Home Health Evaluations by Part-Time Clinician (Nurse Practitioner or Physician Assistant) for Signify Health. Conduct 45-minute visits in patients' homes including medication/history review, physical evaluation, and optional diagnostic tests (e.g., spirometry, eye exams, bone density screening) per health plan orders. Use an iPad-based clinical workflow; no prescribing or altering current treatment. Flexible to travel locally within licensed states and to perform virtual visits as needed. Primary location coverage includes Augusta, ME and Pittsburgh, PA; ideal candidates hold active licenses, board certification, and multi-state licenses (preferred).
Required Qualifications
- Active, unrestricted license(s) in coverage area(s)
- Board certification (required)
- Multi-state licenses (preferred)
- Ability to cover primary locations: Augusta, ME; Pittsburgh, PA (travel locally)
- Able to conduct in-home health evaluations, including history review, physical evaluation, and diagnostic tests as indicated by health plan
- Part-time schedule with anticipated weekly hours around 40
- Proficiency with iPad and clinical workflow tools
- Able to travel locally within licensed states and provide virtual visits as needed
- NPs and PAs with appropriate certifications and scope of practice
