Physician Assistant

Part-time Clinician (Nurse Practitioner or Physician Assistant) to provide in-home health evaluations for Signify Health, a CVS Health company. Responsibilities include conducting ~45-minute home visits that cover medication and medical history review, physical evaluation, and optional diagnostic tests (e.g., spirometry, diabetic eye exams) as ordered by the patient’s health plan. Clinicians use an iPad and a clinical workflow; clinicians are not required to prescribe medications, order labs, or alter current treatment plans. The role involves visiting people in their homes, engaging with members to bridge gaps in care, offering flexible, comprehensive, and personalized health evaluations, and traveling locally as needed with virtual visits as necessary. Primary locations include Augusta, ME and Boise, ID, with the ability to travel within licensed states.
