Physician Assistant

Part-time clinician (Physician Assistant or Nurse Practitioner) to perform in-home health evaluations for Signify Health at CVS Health. Responsibilities include 45-minute home visits with health-question discussions, medication and medical history review, physical evaluation, and optional diagnostic tests (e.g., spirometry, diabetic eye exams, bone density screening) as indicated by the health plan. Use an iPad and a clinical workflow; no prescribing, ordering labs, or changing current treatment during visits. Travel locally within licensed states as needed and conduct virtual visits when required. Primary locations include Augusta, ME, with the ability to travel to Everett, WA. Requires active, unrestricted licensure and board certification; multi-state licenses preferred. Part-time position with anticipated weekly hours around 40 and benefits included.
