Job Summary
Part-time Clinician (Nurse Practitioner or Physician Assistant) to provide In-Home Health Evaluations across a national Signify Health network. Conduct 45-minute home visits including medication and medical history review, physical evaluation, and optional diagnostic tests as ordered by health plan. Use iPad workflow; not required to prescribe, order labs, or alter current treatment. Travel locally within licensed states; some virtual visits may be conducted. Primary locations include Augusta, ME and Houston, TX. Requires active, unrestricted licenses, board certification, and ability to cover primary locations; part-time role with benefits and malpractice coverage.
