Job Summary
In-home NP clinician role with Signify Health (CVS Health) conducting pre-scheduled in-home health evaluations for Medicare Advantage/Medicaid members in the Springfield MA region and surrounding metro area. Responsibilities include conducting in-home visits (approximately 45 minutes), reviewing medication and medical history, performing physical evaluations, and completing diagnostic tests as ordered by health plans. Requires maintaining active state NP licenses, board certification, and multi-state licensure (preferred), with ability to travel locally up to 25%. Part-time position with hourly base pay and incentive structure, plus malpractice coverage and benefits. Emphasis on connecting with patients in their home setting, educating members, and adhering to DPS product requirements and video-visit training as needed.
Required Qualifications
- board-certified nurse practitioners
- In-home Health Evaluations experience (preferred)
- Maintain active state licenses in coverage areas (FNP, AGNP preferred)
- Maintain DPS training and pharmacologic/diagnostic capabilities
- ability to travel locally up to 25%
- part-time role with 40 hours anticipated
- diploma/degree in nursing with NP certification
- Medicare/Medicaid experience (preferred)
