Job Summary
Conduct thorough clinical assessments, including history-taking and physical examinations, while identifying patient and family needs regarding preventative care, medications, and dietary restrictions. Develop and implement treatment plans, order therapeutic regimens, and interpret diagnostic data to monitor a diverse caseload in collaboration with physicians. Manage patient education on disease entities and initiate referrals to community services for a continuum of care. Evaluate the effectiveness of care plans and adjust protocols based on changing patient status. Maintain accurate records and foster professional relationships with Swuc Staff to facilitate communication and ensure effective health strategies.
Required Qualifications
- Nurse Practitioner or Physician Assistant (NP/PA)
- Competent and thorough clinical assessment
- Ability to develop, implement, and evaluate effective health strategies for patients
- Diligence in maintaining accurate records
- Ability to develop and maintain productive, professional relationships with Physicians and staff
- Ability to perform history-taking and physical examinations thoroughly and independently
- Ability to identify the psycho-social and cultural needs of patients and families
- Ability to identify patient and family needs with regard to preventative care, disease entity, medications, dietary restrictions, and other therapeutic regimens
- Ability to order and interpret therapeutic regimen
- Ability to collect and interpret database
- Ability to consult with physicians when appropriate
- Ability to develop a problem list from assessment data
- Ability to establish optimum goals considering the physical and emotional status of the patient
- Ability to plan remedial actions within the framework of the protocol or by collaboration with the physician
- Ability to collaborate regularly with the physician and other members of the healthcare team
- Ability to refer problems when necessary to the physicians
- Ability to demonstrate the ability to develop a treatment plan to meet individual and family needs
- Ability to monitor a diverse caseload of patients in collaboration with the physicians
- Understanding of professional limitations
- Ability to participate in patient care conferences and provide guidance and implementation of care
- Ability to coordinate care of the designated patient population to review the reports, consultations, and recommendations for other healthcare professionals
- Ability to initiate plans, medications, and/or treatment regimens within the guidelines or with a physician consultation when a patient's condition so indicates
- Ability to perform special procedures as indicated by the clinical setting
- Ability to provide, review, and reinforce patient education regarding preventative care, disease entity, medications, dietary restrictions, and other therapeutic regimens
- Ability to initiate referrals to community services to provide a continuum of client services
- Ability to determine the effectiveness of plans on the basis of pertinent data collected
- Ability to evaluate revision and plan of care to meet changing needs
