Job Summary
Provide specialized Physical Medicine and Rehabilitation (PM&R)/Physiatry services as a rehab consultant at a designated skilled nursing facility or post-acute rehab center. Represent CRC through clinical leadership and collaboration with the interdisciplinary care team, including PCP teams and PT, OT, and SLP staff. Manage conditions common in SNFs such as CVA, progressive neurological conditions, and postoperative orthopedic surgeries, while assessing barriers to therapy including joint pain, spasticity, and neurogenic bowel or bladder. Track therapy patient progress and attend multidisciplinary meetings to bridge the gap between therapy and internal medicine. This 16-hour part-time W2 role is located in Farmington, ME, with weekday hours typically between 9 AM and 4 PM.
Required Qualifications
- Nurse Practitioner/APRN/PA License
- Maine License
- Ability to work in the U.S.
- NP/APRN/PA
- Reliably commute or planning to relocate before starting work
