Job Summary
Conduct primary care and family medicine patient management across the care continuum in a 100% outpatient practice setting. Deliver comprehensive care Monday through Friday with minimal call, utilizing EPIC EMR with DAX Copilot for documentation support. Maintain unrestricted Illinois licensure and board certification as a Nurse Practitioner or Physician Assistant while managing specialty referrals and prior authorizations through the dedicated team. Required Qualifications
- Unrestricted Illinois license prior to start
- Board Certified as either an APRN or PA
- Master's degree in Nursing
- Licensed in the state of practice as Advanced Practice Registered Nurse
- Basic Life Support (BLS) certification
Desired Qualifications
- Primary Care/Family Medicine experience
- Doctoral degree in Nursing
- Experience in management of patients across the care continuum
- Board certification from the National Certification Corporation
- Controlled Substance license in state of practice
- Federal DEA license
- Neonatal Resuscitation Program (NRP) certification
- Bachelor's Degree in Nursing PLUS 5 years relevant experience
