Job Summary
Conduct home health visits for geriatric patients, managing adult wellness and chronic disease care. Collaborate with medical assistants, front desk staff, and leadership to provide comprehensive care. Maintain active Florida state medical licensure and hold DEA, BLS, and ACLS certifications. Requires primary care experience in geriatrics and comfort with EMR technology. Schedule includes Monday through Friday, 8:00 am to 5 pm, with no call rotation. Bilingual ability is a plus.
Required Qualifications
- Active or applied for Florida state medical license
- Primary Care experience - Geriatrics
- Comfortable with EMR and tech-savvy
- DEA and BLS/ACLS
- Patient-focused, friendly, and willing to collaborate in a team environment
- Clinical responsibilities to include the ability to see all adult, wellness, and chronic care disease management
