Job Summary
Conduct in-home health evaluations lasting approximately 45 minutes, performing medication and medical history reviews, physical assessments, and diagnostic tests such as spirometry and diabetic eye exams. Utilize an iPad and clinical workflow for each visit to connect one-on-one with patients, answer health questions, and provide education without prescribing medicine or altering treatment regimens. Work as a clinician colleague within a national network, fulfilling specified role requirements while remaining flexible to travel locally and conduct virtual visits as needed. Perform duties in diverse private residences, including homes with pets or allergens, while maintaining safety and quality standards.
Required Qualifications
