Job Summary
In-Home Visits role for a Part-Time Clinician (Nurse Practitioner or Physician Assistant) with Signify Health/CVS Health. Conduct 45-minute in-home health evaluations including medication/medical history review, physical evaluation, and optional diagnostic tests (e.g., spirometry, eye exams). Use iPad and clinical workflow; no prescribing, lab orders, or treatment changes. Travel locally within licensed states; clinical visits at members’ homes; some virtual visits. Primary workplace is members’ homes; environments may include pets or irritants. Requires active license(s) and board certification; multi-state licenses preferred. Anticipated 40 hours weekly; part-time role with benefits.
