Job Summary
Conduct in-home health evaluations lasting approximately 45 minutes, including medication and medical history reviews, physical assessments, and diagnostic tests like spirometry or diabetic eye exams when ordered. Utilize an iPad and clinical workflow for each visit while engaging one-on-one with patients to answer health questions and bridge care gaps. Travel locally and within licensed states as needed, and perform virtual visits when required. Work part-time with an anticipated weekly schedule of 29.5 hours. Candidates must be comfortable performing duties in diverse home environments that may contain pets, secondhand smoke, or airborne allergens.
Required Qualifications
- Active, unrestricted license(s) in coverage area(s)
- Completion of 2080 hours of post-graduate collaborative practice hours as an NP or PA in an integrated clinical care setting
- Filing of the appropriate attestation with the state of Minnesota
- Board certification
- Ability to comfortably and safely perform duties in homes that may contain household pets, secondhand smoke and other potential airborne allergens or irritants
