Job Summary
Conduct in-home health evaluations lasting approximately 45 minutes, including medication and medical history reviews, physical assessments, and diagnostic tests such as spirometry or diabetic eye exams. Utilize an iPad and clinical workflow for each visit while connecting one-on-one with patients to answer health questions and provide education. Perform virtual visits as needed and deliver comprehensive diagnostic and preventive services across a defined coverage area. Maintain active, unrestricted multi-state licenses and board certification to fulfill role requirements. Travel up to 100% of the time, potentially involving overnight stays, car rentals, or flights within licensed states. Work full-time hours with a 40-hour weekly schedule.
Required Qualifications
- Nurse Practitioner or Physician Assistant
- Active, unrestricted license(s) in coverage area(s)
- Board certification
- Able to obtain and maintain multi-state licenses in coverage area, as required
- Able to cover all counties within state and flex to other areas of business need
- Must be able to comfortably and safely perform duties in homes that may contain household pets, secondhand smoke and other potential airborne allergens or irritants
