Job Summary
In-Home Health Evaluations for Signify Health (CVS Health) Part-Time Clinician (PA or NP). Conduct 45-minute visits in members' homes, including medication/history review, physical evaluation, and optional diagnostic tests (e.g., spirometry, eye exams) as ordered by health plans. Use an iPad and clinical workflow; no prescribing or altering current regimens required. Travel locally within licensed states; virtual visits when needed. Primary workplace is the member's residence, with emphasis on delivering personalized health evaluations in diverse home environments. Candidates must hold an active license, board certification, and be able to cover Merrimack County, NH; benefits offered for part-time employees.
