Job Summary
Part-time clinician (Nurse Practitioner or Physician Assistant) to provide in-home health evaluations for health plan members in Marshall, MN. Responsibilities include medication and medical history review, physical evaluation, and, when ordered by the health plan, additional diagnostic tests (e.g., spirometry, diabetic eye exams, bone density screening). Use an iPad and a clinical workflow; not required to prescribe medications, order labs, or alter current treatment. Travel locally as needed; conduct virtual visits as needed; deliver Diagnostic & Preventive Services; provide education and support in patients' homes.
Required Qualifications
- Active, unrestricted license(s) in coverage area(s)
- Board certification (required)
- Multi-state licenses (preferred)
