Physician Assistant, In-Home Visits

In-Home Health Evaluations conducted by Part-Time Clinician (PA or NP) visiting members at their homes; 45-minute visits include medication/medical history review, physical evaluation, and optional diagnostic tests (e.g., spirometry, bone density screening) as ordered by the health plan. Uses an iPad with a clinical workflow; no prescribing, lab orders, or treatment changes required. Requires active unrestricted license, board certification, and preferably multi-state licenses; travel locally within licensed states and occasional virtual visits may be conducted. The role supports CVS Health/Signify Health’s mission to bring personalized health evaluations to people in their homes; environment includes diverse home settings and potential allergens. Qualifications also include the ability to perform home visits and coordinate care within the patient’s plan, with flexible scheduling as part-time employment.
