Job Summary
Conduct 45-minute in-home health evaluations, including medication reviews, physical assessments, and diagnostic tests like spirometry and diabetic eye exams. Utilize an iPad-based clinical workflow to connect one-on-one with patients, reviewing medical history and answering health questions without prescribing medication or altering treatment regimens. Travel locally within licensed states to visit millions of people nationwide, fulfilling all Signify Health services including preventive care products. Perform duties safely in diverse home environments that may contain pets or allergens, while conducting virtual visits as needed.
Required Qualifications
- Active, unrestricted license(s) in coverage area(s)
- Board certification (required)
- Able to cover the following primary locations: New London and surrounding area
- Be flexible to travel locally and within licensed states, as needed
- Conduct virtual visits as needed
- Candidates must be able to comfortably and safely perform their duties in homes that may contain household pets, secondhand smoke and other potential airborne allergens or irritants
