Job Summary
Deliver comprehensive primary care to a set patient panel, including office and telehealth visits for routine and acute needs. Complete comprehensive annual wellness visits and health risk assessments while working closely with physicians, community health workers, case managers, and pharmacists to deliver high-quality outcomes. Practice with a continuous-improvement mindset by following evidence-based guidelines and helping innovate care models that solve real problems for patients. Meet high benchmarks on quality metrics and outcomes, including patient experience, STARS/HEDIS, and utilization. This role supports a value-based care model focused on older adults in rural Western North Carolina, featuring smaller panels of approximately 15 patients per day, longer appointment times with built-in charting, and AI-powered dictation support.
Required Qualifications
- Certified Physician Assistant
- Active, non-probationary North Carolina license (or eligibility to license in NC)
- Minimum of two years' experience in a primary care specialty (internal medicine, geriatrics, or family medicine)
- Experience with HCC (Hierarchical Condition Categories) documentation, ICD-10 coding, and Health Risk Assessments (HRAs)
- Proficiency with EHRs and multiple software platforms, including spreadsheets and data entry/navigation
- Strong organizational skills and the ability to manage multiple priorities
- US work authorization
