Job Summary
Provide onsite primary care services to older adults living in Senior Living Communities; develop and execute person-centered care plans; collaborate with Medical Director to address practice issues; conduct monthly Chronic Care Management visits; lead clinical team huddles; coordinate with senior living management and care teams to improve patient outcomes. Experience as a Physician Assistant, preferably with geriatrics, and ability to travel within the Phoenix area. Strong collaborative and communication skills; comfortable with tech-enabled care delivery tools; hybrid in-office/remote schedule.
