Job Summary
Conduct in-home health evaluations for Medicare Advantage, Medicaid, and commercial plan members, performing medication and medical history reviews and physical assessments in patient homes. Complete an average of 6-9 visits daily using pre-populated, patient-specific forms, focusing on identifying barriers to care and educating members on treatment plans without prescribing medicine or altering existing regimens. Earn weekly compensation of $1800 to $3000 based on a base rate of $70 per completed assessment, with additional pay-per-test incentives and market multipliers available. Work flexible shifts Monday through Sunday as an independent contractor with short-term or long-term availability, supported by malpractice coverage and travel benefits including airfare, hotel, and rental car.

- State License Required
- Independent Contractor, 1099 status
