Physician Assistant for In-home Visits

Contract Physician Assistant to conduct in-home health evaluations for Medicare Advantage, Medicaid, and certain commercial plan members. Conduct in-home visits focusing on overall health and barriers to care, with pre-scheduled visits, in-home health plan member assessments, and potential diagnostic/test incentives. Independent Contractor arrangement with flexible schedule, shifts Monday–Sunday (no nights/weekends required), airfare/hotel/rental car support where eligible, malpractice coverage, and the opportunity to earn via base rate per completed assessment plus market multiplier incentives. State PA license required; board-certified PA/PAAs preferred. Location: Appleton, WI (in-home visits).
