Job Summary
Conduct in-home health evaluations for Medicare Advantage, Medicaid, and commercial members, performing medication and medical history reviews alongside physical assessments. Identify barriers to care and educate patients on treatment plans using pre-populated, patient-specific forms. Complete an average of 6-9 visits daily at $70 per assessment, with additional pay-per-test incentives for qualifying diagnostic and preventive procedures. Earn weekly compensation ranging from $1,800 to $3,000, with rates potentially increasing up to 160% via the Market Multiplier. Maintain a State License (PA-C) and obtain a Medicaid License or willingness to enroll. Work flexible shifts Monday through Sunday, with short-term or long-term contract options available.
