Job Summary
Conduct in-home health evaluations for Medicare Advantage, Medicaid, and commercial plan members, focusing on overall health, medication reviews, and physical assessments. Perform pre-scheduled visits with health plan members to identify care barriers and educate patients on treatment plans. Complete an average of 6-9 visits daily using provided patient-specific forms without prescribing medicine or altering existing regimens. Earn weekly compensation ranging from $1800 to $3000 based on a base rate of $70 per assessment, with potential multipliers and pay-per-test incentives. Requires a state license and Medicaid enrollment willingness; available Monday through Sunday with flexible scheduling. Independent contractor status with malpractice coverage and travel support including airfare and hotel.
