Job Summary
In-home Health Evaluations for Medicare Advantage, Medicaid, and certain commercial plan members conducted by a board-certified Physician Assistant (PA) contractor. Perform in-home visits focused on overall health and identifying barriers to care, using pre-populated patient-specific forms. Earn an attractive earnings potential with a base rate per completed assessment and possible Market Multiplier incentives; average workload supports 6-9 visits daily. Flexible schedule with shifts Mon-Sun and no nights/weekends required. Independent Contractor (1099) status; Medicaid license enrollment supported. States license required. Visits involve health history review, physical evaluation in the patient’s home, and do not involve prescribing medications, ordering labs, or altering current treatments. Pre-scheduled visits with health plan members; malpractice coverage provided; airfare/hotel/rental car available dependent on eligibility. Area: Manitowoc, WI.
