Job Summary
Conduct in-home health evaluations for Medicare Advantage, Medicaid, and commercial plan members, focusing on overall health, medication reviews, and physical assessments. Perform pre-scheduled visits with health plan members to identify care barriers and educate patients on treatment plans without prescribing medicine or altering existing regimens. Complete an average of 6-9 daily visits, earning up to $3000 weekly with a base rate of $70 per assessment and a market multiplier. Requires a state license, Medicaid enrollment willingness, and availability Monday through Sunday. Independent contractor status with flexible scheduling and travel support including airfare, hotel, and rental car.
