Physician Assistant

Part-time clinician role for Signify Health (CVS Health) as a Physician Assistant or Nurse Practitioner providing in-home health evaluations. Visits last about 45 minutes, include medication and medical history review, a physical evaluation, and, if ordered by the health plan, additional diagnostic tests (e.g., spirometry, bone density screening). You’ll use an iPad and a clinical workflow; no prescribing, ordering of labs, or altering current treatment plans. Responsibilities include connecting with members at home, building trusted relationships, traveling locally within licensed states as needed, and conducting virtual visits when required. Primary workplace is the member’s residence, with environments that may include pets, secondhand smoke, and other potential irritants. Part-time benefits available. Requirements: active, unrestricted licenses, board certification, and (preferred) multi-state licenses. Anticipated weekly hours: 29.5. Locations include Penobscot, ME (US). The role focuses on providing Diagnostic & Preventive Services through Signify Health’s in-home program and may involve travel to members’ homes and occasional virtual visits.
