Job Summary
Part-time clinician (Nurse Practitioner or Physician Assistant) to provide in-home health evaluations, engaging with people in their homes, performing medical history review, physical evaluation, and, if ordered by health plan, additional diagnostic tests. Role uses an iPad workflow; not required to prescribe meds or alter current treatment. Travels locally within licensed states; conducts virtual visits as needed; primary workplace is the private residence of health plan members. 29.5 hours/week anticipated; part-time position with benefits and malpractice coverage for Signify Health employees.
