Job Summary
Physician Assistant at Huron Valley PACE in Monroe, MI. Provide comprehensive assessments, diagnoses, and treatment plans, supporting health, independence, and well-being of older adults. Work within an interdisciplinary care team to coordinate patient-centered care under the guidance of the Medical Director. Responsibilities include health histories and exams, ordering and following up on lab tests, initiating and coordinating care under physician-approved protocols, providing emergency care when needed, collaborating with ER and hospital staff to prevent unnecessary hospitalizations and facilitate discharge, acting as liaison with physicians and other healthcare professionals, coordinating care with contracted services and outside providers, participating in IDT meetings, and educating participants and families on preventive health maintenance and chronic condition management. Adhere to regulatory requirements and ensure quality improvement and compliant documentation.
Required Qualifications
- Current license and certification to practice as a Physician Assistant in the State of Michigan
- Satisfactory completion of a Physician Assistant program
- 5–7 years of experience with at least 1 year of primary care experience with frail, elderly, or long-term care populations
- Experience working in a PACE (Program of All-Inclusive Care for the Elderly) facility
- Proof of clearance for all communicable diseases prior to participant contact
